As part of the application, please email your proof of residence to Kelly Heiss. We accept gas, water and electric bills in the parent/guardian's name. Please make sure both the service and mailing address are visible, usually this means sending a copy with the payment coupon attached. If you do not have a gas, water or electric bill in your name, please call 937-349-3731 ext. 1200 or email Kelly Heiss.
