All 7th graders (if you have NOT already had one) MUST have an ADDITIONAL Tdap NEW 2016 – 2017 school year is MCV4 (Meningococcal). You may call the Union County Health Department @ 937-645-2058 to schedule an appointment, or go through your family doctor. Health Department Clinic Hours are Monday - Thursday from 8 - 4. They are closed from 12 - 1 for lunch. Proof must be sent in by Friday, September 3, 2021.
