Process for Obtaining a Student Work Permit

• Download the Work Permit Form [PDF Document]
• Fill out the appropriate student section.
• Have your employer fill out the employer section.
• If you have not had a physical this year, you will need one prior to approval.
• Return the completed application and proof of physical to the FHS office.
