Enrollment Policy - District Residents
We welcome you into our district. Below is the following information for enrolling your new students into our district.
The district has Centralized Enrollment. Therefore, you must register your child at the Board of Education office which is located just inside the Middle School front entrance. Our hours are 7:30 - 4:00 P.M. and please ask for either Chris Logan or Marilyn Reames. If you need to call, please contact us at 937-349-3731 ext. 1209 for Chris or ext. 1200 for Marilyn. Listed below is the necessary information needed to enroll your child into our district.
Mandatory Documentation: YOU MUST have these documents at the time of enrollment. We cannot contact school districts to get this information during the enrollment process. This information is important and must be submitted prior to your child starting school.
- An enrollment packet will need to be completed. The packet can be downloaded here [PDF Format] or completed at the Board of Education office.
- Child's Birth Certificate (This needs to be the original. We do not accept the "foot prints" which are produced at the hospital.)
- Immunization Records: Please make sure this is the most current record!
- Proof of Residency (This must be a utility bill, rental agreement, or tax statement). If you are living with someone, they will need to bring their proof of residence and will need to complete a Residence Affidavit at the time of enrollment.
- Custody papers, divorce decree, or separation papers
OTHER ENROLLMENT FORMS: Once you have completed the mandatory documentation at Central Office, you will then need to contact the individual building.
